Although no longer circulated, this safety questionnaire is available as a tool which could, for example, be completed and submitted yearly to your Departmental Safety Committee as part of an annual report.

Please note that questionnaires will still be circulated for specialist safety, e.g. Radiations (both Ionising and Non-ionising).
