Working from home (‘teleworking’)

About the form

Form PD/WFH is a formal proposal to work from home under the Flexible Working Policy.

Those exercising their right to apply to work flexibly under the Flexible Working Regulations (implemented under the Employment Act 2002 and introduced by the Government from 6 April 2003) should also complete form FLEXAF.

A request to work from home includes a formal risk assessment of the home working environment.

Guidance notes on working from home and information on conducting a risk assessment are available and must be considered before completing the form.

Filling out the forms

All sections of the forms must be completed.

The form should be completed in consultation with the immediate manager. When signed by both parties and the head of the Institution, a copy must be forwarded to the appropriate HR Consultant/Officer.
