About the form

Form PD18 should be used to accompany applications by CV.

If you are applying for a support staff post, please use form PD17 Application for Employment.

Filling out the form

The PD18 is broken down into three parts:

  • Part I contains information that will be used in the selection process. This should be completed by all applicants.
  • Part II contains information for additional vetting. This is required for some posts and you will be advised accordingly. For example:
    • You would complete Section A if you are applying to a post in a biological facility.
    • You would complete Section B when a Criminal Records Bureau check is required.
  • Part III is optional; however, completion of this section is encouraged of all applicants and provides equal opportunity information. This information is not used as part of the selection process.
