Forms to request flexible working

Forms should be completed as set out below.

Non-officers

CHRIS/50 should be completed, attaching the following documentation:

Temporary requests

  • CHRIS/50 and a letter (see templates below).

Permanent requests or where the application is under the Flexible Working Regulations

  • CHRIS/50 and a FLEXAF form (see guidance on completion below)

Officers

Forms should be completed as follows:

Temporary requests

Your HR Business team will then prepare a letter granting leave under DII6c (leave can be granted for up to 5 years at a time).

Permanent requests or where the application is under the Flexible Working Regulations

  • FLEXAF form should be completed by the officer (see guidance on completion below).

Your HR Business team will then liaise with you concerning the relevant process to set up a part-time office.

Notes on forms

FLEXAF form should be completed by members of staff wishing to apply to work flexibly on a permanent basis or exercising their right to apply to work flexibly under the Flexible Working Regulations (implemented under the Employment Act 2002 and introduced by the Government from 6 April 2003). Guidance notes on eligibility are available.

FLEXAF/DECISION form should be completed by the Head of Institution or authorised signatory after a meeting with the employee to review the application.

Obtaining a copy of the forms

All sections of the forms must be completed. The completed forms together with a letter setting out the reasons for the request should be sent to the appropriate HR Business Manager or HR Adviser. Forms FLEXAF and FLEXAF/DECISION may be downloaded in PDF, Word or Rich Text Format.
