Flexible Working Forms

About the form

Form FLEXAF should be completed by members of staff wishing to apply to work flexibly on a permanent basis or exercising their right to apply to work flexibly under the Flexible Working Regulations (implemented under the Employment Act 2002 and introduced by the Government from 6 April 2003). Guidance notes on eligibility are available.

Form FLEXAF/DECISION should be completed by the Head of Institution or authorised signatory after a meeting with the employee to review the application.

Filling out the forms

All sections of the forms must be completed. The completed forms together with a letter setting out the reasons for the request should be sent to the appropriate HR Business Manager or HR Adviser.
