About the form

Institutions should use this document for recording discussion at the consultation meeting with a member of staff about the possible ending of their open-ended contract. For staff on a fixed-term contract please use the CHRIS/43 form.

Returning the form

A copy of the completed form must be sent to the Human Resources Division so that it:

• Is aware of the meeting that has taken place and has a record of this on the individual's personal file.
• Can assist with redeployment where requested.
• Has the account code for any redundancy payment which may be due.
